What is Vitamin B12
Vitamin B12 is a water-soluble vitamin, also known as cobalamin. Vitamin B12 is naturally present in some foods, added to others, and available as a dietary supplement and a prescription medication. No toxic effects of vitamin B-12 have been identified, even when it is administered intramuscularly at 300–3000 times the recommended dietary allowance. For this reason, no upper tolerable level for the vitamin has been established.
Vitamin B12 Deficiency
Vitamin B12 deficiency is common, mainly due to limited dietary intake of animal foods or malabsorption of the vitamin. Vegetarians are at risk of vitamin B12 deficiency as are other groups with low intakes of animal foods or those with restrictive dietary patterns. Malabsorption of vitamin B12 is most commonly seen in the elderly, secondary to gastric achlorhydria. Unlike other vitamins, the absorption of vitamin B12 depends on a protein produced in your stomach, called gastric intrinsic factor (GIF). GIF binds to vitamin B12, so that you can absorb it into the blood. People who don't produce enough GIF can become deficient. Other people at risk of deficiency include those who have had intestinal surgery, including weight loss surgery. Those with diseases that affect the gut, such as Crohn's disease or celiac disease, are also at risk.
Vitamin B12 and Health
Vitamin B12 deficiency is characterized by megaloblastic anemia, fatigue, weakness, constipation, loss of appetite, and weight loss. Neurological changes, such as numbness and tingling in the hands and feet, can also occur. Additional symptoms of vitamin B12 deficiency include difficulty maintaining balance, depression, confusion, dementia, poor memory, and soreness of the mouth or tongue. The neurological symptoms of vitamin B12 deficiency can occur without anemia, so early diagnosis and intervention is important to avoid irreversible damage. During infancy, signs of a vitamin B12 deficiency include failure to thrive, movement disorders, developmental delays, and megaloblastic anemia. Many of these symptoms are general and can result from a variety of medical conditions other than vitamin B12 deficiency.
